Has anyone heard of a monk/cleric character? If so where?

Don't know, but I do recall that Bob Salvatore originally wanted to transition away from Drizzt the drow ranger/fighter to do a "The Monk Quintet" series, but TSR told him no, so he changed it to "The Cleric Quintet", and just had the cleric marry a monk, instead. That's sounds close!

It just seems that with many of the good deities have their clergy and monestaries together. So how about a monk who can cast divine magic?

Realms-wise there are a few orders of monks that can multiclass freely to clerics, namely the Sun Soul monks (out of Waterdeep: City of Splendors) and the Monks of the Yellow Rose (FRCS). Also the Broken Ones who follow Ilmater (FRCS). Additionally, any monk can also train to be a Sacred Fist (Complete Divine) and I don't think there's anything barring that Prestige Class from the Realms canon wise.

Mechanically speaking the two have very strong synergies as they're both based off of Wisdom for a majority of features. Monks use it for practically everything outside of combat and Clerics use it for all their spellcasting. Where they get into trouble is mitigating the MAD (multiple attribute dependency) of the multiclassed (using v3.5 rules) character. A Monk wants good DEX for combat, a great WIS for his abilities, a decent CON for HP and Fortitude, and a medium (10 starting) for Strength. There are feats that help him out though such as Zen Archery (use Wisdom in place of Dex for Ranged touch attacks), Weapon Finesse (use Dex in place of Str for melee attacks), Superior Finesse (use Dex in place for Str for melee damage rolls). And of course you want feats like Extra Turning, Quicken Spell, Divine Metamagic [Quicken Spell], Persistant Spell, etc.... that really boost your spellcasting capabilities.

In 4th Edition, a Hybrid is pretty awesome as Monks are SUPER quick and elusive, deal decent damage, use implements, have attacks that his multiple foes, etc. Couple that with Cleric healing, boost granting to allies, Radiant attacks, and a decent deity and you're set.

The Sohei class in Oriental Adventures (3e) was a blend of monk and divine caster.

Ilmater for mine always seems a logical choice for a monk/cleric type , given the need for a lawful god and character and the need for alot of discipline and even inner peace helping the downtrodden or slaves in some areas like Calimshan. Any of the Triad works, but Ilmater is the pick.
Kelemvor is so new that monk/clerics dedicated to rapidly ending undead perversions could spring up.
Azuth may have some like the magehound prestige class from the Shining South in Halruua , Bane may even evolve some monk/clerics out of the east.
A few left options are Hoar ( as a king/queen of vendetta) , Red Knight ( war tactics ) , Tiamat , Gorm Gulthyn , Gaerdal Ironhand and Arvoreen.

My understanding is that Monks are already a subset of Clerics and Priests; there are all sorts of intermediate subclasses (and religious orders) which blur the boundaries and might call themselves by any of these names.

If the question is about Monks who are basically the D&D equivalent of Shaolin martial artists, then the answer is yes ... there is the "optional" AD&D 1E Monk class (along with several of the 1E Oriental Adventures classes), there are many AD&D 2E warrior and priest kits, there are any number of 3E/3.5E Prestige Classes, and there are countless dual-class or multiclass possibilities. There are "fighting Monks" who use all sorts of combinations of unarmed combat, armed combat, priest spells, arcane spells, ninja skills, psionics, and even gadgets.
